Priority Wait List Request (Enrollment Process)

PROCESS:

 

Families complete a form to be placed on a Priority Wait List and include a months worth of paystubs. Please submit completed form to the receptionist at our district office at 4400 Alma Avenue.  Incomplete forms will not be processed.    

Completed forms and paystubs can also be scanned and emailed to Dr. Jennifer Tomita at [email protected].   

Completed Forms will be processed and families will be notified of the status within 30 days of time of submission.
